1. difinition
- 부신 수질의 크롬친화성 세포(chromaffin cell)에서 발생한 catecholamine 분비 종양
2. Rule of 10
- 10%는 HTN 과 무관
- 10%에서 bilateral
- 10%는 malignant
- 10%는 familial
- 10%는 extra-adrenal
- 10%는 소아에서 발생
3. Sx.
classic triad - HSP (headache, sweating, palpitation)
- 종종 postural hypotension
- HTN (주로 sustained)
- 불안감, 신경질, 체중감소, 시력장애, tremor
4. Dx.
- excessive catecholamine check
24h urine check ①NE와 EPI ②metanephrine과 normetanephrine ③VMA
- tumor location check
CT & MRI
percutaneous FNA는 CIx (∵crisis 초래 가능)
5. Tx.
- 수술로 완치 가능한 HTN !!!! (↓ 아래 약들은 수술 상태를 만들기 위함..)
- α-blocker : phenoxybenzamine (단지 preoperative management 14일정도 투여)
- β-blocker : α-blocker로서 혈압을 떨어뜨릴 수 없을때 (★단독으로 주어서는 안 된다! 꼭 α-blocker 후에 투여!!)
- 그밖에 CCB나 ACE inhibitor도 고려
수술 직후 hypotension 주의!!
